Mahmut Nedim Doral, Gazi Huri, Egemen Turhan, Defne Kaya, Gürhan Dönmez, Kadir Büyükdoğan, Mustafa Sargon While the Achilles tendon (AT) is the strongest and thickest tendon in the human body, it is also the most common tendon to rupture. It begins near the middle of the calf and is the conjoint tendon of the gastrocnemius and soleus muscles. Though numerous non-operative and operative methods have been described, there is no universal agreement about optimal management strategy of acute, total AT rupture. Since endoscopy-assisted percutaneous AT repair allows direct visualization of the synovia and protects the paratenon, which is important in the biological healing of the AT, this technique is a reasonable treatment option in AT rupture. Aim of this review was to discuss details of endoscopy-assisted percutaneous repair of Achilles tendon rupture and present experience.

Achilles Tendon Ruptures and Modern Treatment Approaches

The Achilles tendon (AT) is recognized as the strongest and thickest tendon in the human body. Despite its robust structure, it remains the most frequently ruptured tendon. Anatomically, it originates near the middle of the calf as the conjoint tendon of the gastrocnemius and soleus muscles. While numerous operative and non-operative methods have been developed over the years, a universal consensus on the optimal management strategy for acute, total AT ruptures has yet to be established.

Endoscopy-Assisted Percutaneous Repair Technique

Endoscopy-assisted percutaneous AT repair has emerged as a highly effective surgical alternative. This technique offers distinct advantages by allowing direct visualization of the synovia, ensuring a more precise intervention. One of its most critical benefits is the protection of the paratenon, a structure that plays a vital role in the biological healing process of the tendon.

Key Advantages of the Endoscopic Approach

  • Direct Visualization: Enables clear monitoring of the synovia during the procedure.
  • Biological Preservation: Protects the paratenon to facilitate natural healing.
  • Minimally Invasive: Offers a reasonable treatment option compared to traditional open surgeries.

Clinical Significance and Review Objectives

Given its technical advantages, endoscopy-assisted repair is considered a reasonable and sophisticated treatment option for patients suffering from Achilles tendon ruptures. The primary aim of this review is to discuss the technical details of endoscopy-assisted percutaneous repair and to present clinical experiences regarding its application in modern orthopedics.
