Cosmetic concerns represent the primary reason for patient presentations regarding the excision of nevi in the facial region. Patients often perceive facial nevi as aesthetically disturbing due to their raised appearance, pigmentation, or the presence of hair. Additionally, their conspicuousness to others and their location within the shaving area in male patients—where they are frequently subjected to trauma—are significant factors driving the demand for removal.

The Importance of Histological Confirmation

While dermoscopy serves as a valuable noninvasive diagnostic tool in experienced hands, it is crucial to recognize that biopsy remains the only definitive method for confirming dysplasia in suspicious cases. Clinical intuition alone can be misleading; studies indicate that 2.3% of lesions clinically presumed to be benign nevi were histologically diagnosed as malignant tumors, including:

  • Melanoma
  • Basal cell carcinoma
  • Squamous cell carcinoma

Surgical Techniques: Balancing Histology and Esthetics

From a clinical standpoint, only complete scalpel excision of all nevi allows for a full histological evaluation of the lesion. However, the increasing emphasis on cosmetic outcomes and growing clinical workloads has led to the adoption of faster, less traumatic techniques.

Commonly employed methods for diagnosis and treatment include:

  1. Punch biopsy
  2. Shave biopsy
  3. Scoop shave biopsy

It is important to note that the primary objective of cosmetic surgery for nevi is largely the removal of the visible lesion, which is inherently an incomplete removal process.

Optimization of Scarring and Wound Closure

Among the various techniques for removing moles, only elliptical excision or a properly sized punch biopsy can ensure complete removal and facilitate comprehensive pathological evaluation. The geometry of the excision significantly impacts the final cosmetic result.
